Biography

Vivianne Schlam Jakobs, FNP is a board-certified Nurse Practitioner. She received her Bachelor of Science in Nursing from Saint Anthony’s College of Nursing, followed by receiving her Master of Science from the University of San Diego.

She received her Doctorate in Nursing Practice from Walden University and her Post-Master’s Certification from Cal State University, San Marcos. She began her medical career in 2004 and has worked with several medical organizations including CGH Medical Center, Sterling, IL, San Diego Hospice and Palliative Care, Welcome Back Center/Grossmont College, Rady’s Hospital outpatient, HHSA County of San Diego and CDC/USMU San Diego Quarantine Station. She has over 15 years of experience having worked in both inpatient and outpatient settings, and she has experience with hospice and palliative care. Vivianne has promoted better health practices throughout her career, provided medical and nursing advice, and been involved with several scientific studies and surveys to identify public health issues.

Vivianne’s patient philosophy is based on providing competent, empathetic, compassionate, and optimal holistic care to the best of her ability. This stems from the values and beliefs instilled in her during her formative years. Vivianne chose to join TOI because she understands the impact and privilege of being there through every step of the patient’s cancer care journey. Cancer patients are a very vulnerable population and she knows just how much providers can make a difference in their care and quality of life.

Vivianne speaks multiple languages including English, Spanish, Hebrew, and Yiddish. She will be seeing patients at The Oncology Institute’s Chula Vista location.
